Why You Need One

Every client. Every session. No exceptions.

In the UK, tattooing is a regulated activity. Before carrying out any tattoo, you are legally and professionally expected to obtain informed consent from your client. This means your client must understand the procedure, the risks involved, and provide their agreement in writing before any work begins.

A proper consent form does more than satisfy a legal requirement. It documents that you screened for contraindications: conditions such as blood thinning medication, skin conditions, pregnancy, or compromised immunity: that could affect the safety of the procedure or the quality of the result.

If something goes wrong, a well-completed consent form is your primary evidence that you acted professionally and responsibly. It protects you, your business, and your client.

Many tattoo artists, particularly those new to the profession, use inadequate forms or no form at all. This template gives you a professional starting point used by practising fine line artists.
